With the nature of midwifery care taking place outside of the hospital setting, our midwives perform a detailed risk assessment on each client prior to establishing care to ensure that your medical history is supportive of a safe out-of-hospital delivery.
We require each client to watch our onboarding video provided below. This will answer the most common questions that inquiring clients have and provide a general understanding of what midwifery care entails.
After completing the video, please fill out our risk assessment questionare found below. Our risk assessment form will assess if you are a medically-appropriate fit for out-of-hospital care prior to scheduling an appointment.
